NOTICE OF PRIVACY PRACTICES FOR PROTECTED HEALTH INFORMATION
Purpose
To remain compliant with the Health Insurance Portability and Accountability Act (HIPAA).
Policy
It is the policy of Northern Chaplain Services to uphold the Privacy Practices at the facilities where they are operating.
Practice
The patients health information is protected by both federal and state laws. Northern Chaplain Services will comply with the laws that are most protective of their health information and will not release any information about a patient, nor confirm the identity of a patient without the expressed permission of the patient.
